I think rather than having "date proposed," it would be better to have "date enacted" and leave that for the Assembly Speaker to amend after enactment. I'm not sure the date it was proposed matters nearly as much as the date it was enacted.
I definitely think "Proposed by:" and the optional preamble are good suggestions. I'll edit those, as well as requiring the Assembly Speaker to append the date enacted, into the formatting requirements.
